Your EAP member services offer ways to help overcome such issues, and will help you identify problems, develop a plan, and gain referrals to appropriate resources. The results of effective problem management are easy to recognize: better focus, better work productivity, and a clearer path to achieving your personal and professional goals.
Your EAP member services provide a wealth of resources that can help you lead a healthy, happy, and well-balanced life – including emotional health, wellness, and work and life services for you and your covered dependents. And:
- Your EAP is easy to use and paid for by your employer.
- There are no co-payments, coinsurance or deductibles; you will not have to pay MHN counselors for any services covered by your EAP.
- All services are confidential. MHN will only release information as required or permitted by law.
- MHN recognizes the specific needs of its members, and is committed to treating members in a manner that respects their rights. Please read the attached Member Rights and Responsibilities statement, or print it for your reference.

Full and Equal Access Information
You have the right to get full and equal access to health care services covered by your health plan. This is also true if you have a disability, according to the following laws:
- The Americans with Disabilities Act of 1990.
- Section 504 of the Rehabilitation Act of 1973
Emotional Health
When you need a helping hand, a skilled professional can provide focus, direction and support. MHN can help you identify and resolve issues involving:
- Marriage and relationships
- Family conflict
- Stress, anxiety and emotional distress
- Grief and loss
- Alcohol or drug dependency
- Life changes
Eligible members are entitled to up to 5 appointments per incident per policy year. Choose from:
- Face to face appointments with an MHN network provider
- Telephonic appointments - for maximum convenience and anonymity
- Web-video appointments - convenient and easy, but with a more personal touch than traditional telephonic

Online Wellness Resources
Your EAP Wellness Center offers all sorts of assessments, programs and tools to help you achieve your health and wellness goals. Once you register, go to My Plans to assess your health and develop plans to get fit, eat right, manage stress, or stop smoking. Our interactive programs will help you track your progress and stay motivated every step of the way.

Work & Life Services
Childcare problems produce frustration, worry and guilt. They can interfere with a parent's routine, causing missed time from work and canceled plans. When you call MHN for childcare assistance, our care consultants help you:
- Assess your childcare needs
- Understand the differences in cost and structure of the types of childcare available for infants, preschoolers and school-aged children
- Identify and evaluate childcare and special needs resources and providers
Your assigned care consultant will provide contact information for local childcare providers with current confirmed openings across multiple zip codes.

Every year one out of three people in the U.S. is confronted with a legal problem. Many of these problems can be resolved with advice from a licensed attorney before they escalate, saving you time and money and minimizing the stress and anxiety caused by unresolved legal problems.
You are eligible for one initial free 60-minute telephonic or face-to-face legal consultation per separate legal matter, with a network attorney. If you want to retain an attorney or a mediator after the initial consultation, you get 30% off the normal hourly rate (flat fees are likely not discounted).
Our attorneys deal with almost all types of legal matters, including divorce and child custody, contractual and consumer disputes, real estate and landlord-tenant issues, and car accidents and insurance disputes.
(Matters involving disputes or actions between members and their employer, or MHN, are excluded from this plan. Also excluded are matters that, in the attorney's opinion, lack merit. Court costs, filing fees and fines are the responsibility of the member.)

If you have been victimized, we can help. Call right away to receive a 30-minute consultation with a certified consumer credit counselor who can assess your situation, create an action plan and provide the knowledge and tools to implement that plan effectively. If there is a potential of ID theft, we will warm transfer you to an identity recovery specialist who can advise you on how to place fraud alerts, freeze credit, file police reports, and conduct other activities necessary to resolve fraud.

Do you worry about loved ones who are on their own when you're unavailable? MHN's Family Check-in Services are here to help.
- You set up a phone schedule with a check-in consultant so that your unattended child or aging parent receives a phone call at regular, pre-scheduled times.
- If your family member is not reached on the first phone call, the consultant tries two additional times over the next hour for children or over the next four hours for adults.
- If contact is not made on those attempts, the emergency contacts you listed during the registration process are notified immediately.
Getting Started is Easy!
You can register quickly and easily. You will receive a registration kit with information on child and elder safety and how to discuss the program with your loved ones. Your check-in consultant will also review the program with the child or elder, ensuring that everyone involved understands and is comfortable with the service.

Wellness
Our wellness coaching services offer extra support when you're ready to change your life – helping you make a plan, and then offering the support, resources and accountability you need to succeed.
Our certified coaches can help you with all sorts of health issues, including:
- Weight management
- Smoking cessation
- Fitness and exercise
- Stress management
- Overall lifestyle improvement
- Lifestyle support for chronic conditions such as asthma, diabetes, and cardiovascular disease

Because this is a highly personalized program, you and your coach will decide what kind of help you need to reach your goals. You can:
- Schedule follow-up calls with your coach (participants average about four 15-30 minute consultations).
- Reach out as needed via phone, email or IM (coaches are available Monday through Friday, 7:00 a.m. to 8:00 p.m. Eastern time).
- Access multimedia programs, health assessments, menu planners, fitness trackers, and more through a secure, personalized web portal.
- Receive personalized emails and coach-chosen materials.
